The car starts rough, but in a few seconds runs smooth. The scanner indicated that mass airflow sensor needs to be replaced. Should I run a more thorough diagnostic before buying the new sensor?

It sounds like the Mass Sensor is no good for sure by the rough starting. I would go ahead and change that and then run more diagnostic checks.
