Am I ruining my transmission?

Bought a Nissas Sentra XE almost 4 years ago.  It thumps when the
automatic transmission shifts from 1st to 2nd, about 22 mph.  A shade tree
mechanic told me I just need a transmission service.  But I don't drive much,
an average of about 800 miles/year over the last 4 years.

The thump varies from hardly noticeable to very noticeable.  The only other
thing is that I need to pull the shifter firmly into DRIVE position to get it in gear
when first starting the car.  It's not so demanding after I've driven a bit.

I know I should get the service, but there have been other more pressing
repairs.  And fixed income yada yada yada.  

What kind of damage am I doing to my car?   Can I keep putting this off for a
few 100 miles?

The thump might be a bad transmission or engine mount.
