must all 4 run flat tires be replaced at same time?

Since you car is an X drive, BMW is going to say yes. The tires should be all the same size and same amount of wear so it doesn't affect the transmission/transfer case. You can probably get away with replacing just 2 on the same axle (front or back) as long as the others 2 are not very low on tread.

Ditch the run flats and you will be glad you did. Get a tire repair kit and a compressor or a couple of cans of fixaflat and a AAA membership!
