2003 mustang jerks all gears, not when top ending gears

Jerks midgear, low gear in all gears unless I am
really pushing it...

has this vehicle been modified? like has the engine and or trans been replaced recently? it could be as simple as a vacuum line leaking or broken or being in need of a fluid / filter change. if not then you may need a torque converter or even a transmission / rebuild
