Idles Rough But Only When...

Here's a unique one: Car idles roughly ONLY when in bumper to bumper traffic
on highway. Not on local streets. Not even on open spaced highway. Meaning,
this tends to happen when I'm in bumper to bumper George Washington bridge
traffic. Ideas?

I should add that this happened several years ago and the mechanic at that point said they found nothing (I think they just hooked it up for codes) and it was fine until a week ago. No check engines, no other rough idling, just in that situation.
