Sounds like your ABS System is activating. Any warning lights on or flashing when the engine is running?

when going slowly to stop and then applying pressure is when the jerk occurs, but not always

Ok....if there are NO warning lights on or flashing when the engine is running......you might have a Brake Rotor which has warped. When you apply the brakes.....do you feel a shutter in the car? Does the steering wheel wobble? Make sure your Parking Brake is fully released. Make sure your tire pressures are at specs. Get back with results.
