ATF & Raiator coolant

I am an original owner of a  2001 GMC Sierra 1500, V8 (5.3 Vortec) with 131000 miles now. Flushed the Radiator and Transmission 2 times each so far.  Do I need to replace the Radiator or just flush it again? Do I need to replace Transmission Filter as well or just flush the fluid?? Thanks.

Right, is there a problem? Are the two fluids mixing? You need to replace the radiator if that is the case. The automatic transmission cooler inside the radiator has failed.
