1992 4Runner 3.0 V6 EFI engine. Starts great, idles fine, plenty of get up and go but balks when maintaining speed. Is it the fuel pressure regulator?

She has 268,000 miles but has had the head gaskets replaced again 8,000
miles ago and valves done 68,000 miles ago. replaced fuel filter last fall. I am
just lately getting real bad gas mileage, about 12mpg, and it balks when
trying to keep an even speed like riding a bull sometimes at lowest speeds
in traffic. Are these symptoms of a Fuel Pressure Regulator? or something
else. I was told by a mechanic at the dealer that the regulator was likely the
problem.

I would also check the catalytic converter.

Get new irridium spark plugs for mpg. Doesn't cost too much either.

Also z-max fuel additive helped mine. I'm at around 13.8 city and don't drive slow at all.
