Always in ACC mode even without key

I have replaced alt, batt, ignition switch and
cylinder but it stays in ACC mode even
without the key. Had to put a kill switch
under hood.  Any ideas??

Wiring issues, cross connections possible in the main wiring harness. Could be a real bugger to find.
