My 97 suburban blows cold under 60mph

My 97 suburban blows cold air unless I'm over
60mph, then it blows heat.  As soon as I begin to
slow it blows cold again.  As well, I noticed to
defrost only blows on the passenger side.  To top it
all 9ff, the actual is no longer working.  
Suggestions?
