Is there a way to retrofit the back middle seat belt to include a shoulder belt?

Dear julie-- There is hardly anything that you can't do to a car or truck if you are willing to pay the price. Retrofitting those seat belts on your 1985 Toyota 4Runner will require a lot of billable hours from whoever does the work for you. My suggestion would be for you to ask for an estimate at body shops and custom outfitters in your area. Good luck from Bovan.

yes it is possible but you would have to either have an anchor point welded to the roof or have it come from the floor over the back of the seat
