Nissan Pathfinder 1999 Manual Trans

When driving in most gears you press on the gas and the truck starts to rev
really high and then plains out. I will be doing 100 kph and 2500 rpm then push
on gas and it revs up to 4000 and then plains out after a bit.

Sounds like your clutch is slipping. Get it adjusted before it burns out. If it's a real high mileage vehicle, there may be no more adjustment left and will need a new clutch.
