when heat is turned on only low to medium temperature comes out not hot air
, lots of air flow , could suv need rad flash and new rad fluid ?

It is possible it may need a new thermostat, or a heater valve in the heater hose that also sets the temperature of the heat coming into the cabin. Coolant should actually be changed every three years, but I don't know anyone who does that, but that is whats recommended. So a change in fluid and flush is only a plus.
