Rough transmission shifts

I have a 2001 Lincoln Continental with about 136k miles.  Not always but from
time to time the car shifts very roughly, sometimes in the form of a hard clunk
shift or is slow to down shift when slowing down to make a slow turn.

same issue i have it in my 2018 Lincoln Continental,
