Coolant leak

My car has a coolant leak behind the engine. It isn't
big. It drips out . But when my car gets hot the
resovoir starts boiling like a tea kettle with steam
shooting out. The heater fluctuate between
warmish air and cold. What could be the problem?

or a freeze plug..how often do you add coolant/water????
