Its possible the truck was overloaded. Check your shocks too. How bad is it sagging? is there any weight in the bed?

im the second owner i noticed a slight tilt in the rear end when you look from afar, but the shocks are still good, when i looked at the leafs the passenger side does seems to look somewhat stretched out.... hmmm....

Look at the spring forward of the axle and see if it has a wavy shape or any irregularity comparted to the other side, If it does you'll have to buy a set.
