What causes atf to be in the coolant

There probably is a transmission cooler inside the radiator. When that cooling loop gets compromised this creates a path for ATF to get into the engine coolant and vice versa. Coolant can also get into the transmission and that is a very bad thing. Most likely you will have to replace the radiator, flush the cooling system and if there is any sign of coolant in the transmission, change it's fluid as well.
