Coolant being forced out of radiator once it is warmed up. I have put a new cap on. What is going on?

Not sure where to start. I top off raditor almost everytime I go someplace. Can not find any leaks. If hot, coolant is coming out of upper radiator hose. It has a odd looking device attached to bottom of the hose. I have replaced radiator since I found it leaking. I know have a knocking from engine on left side of block, same side that gets the coolant sprayed onto it. Any ideas?

Sorry I only replaced the radiator cap. Seems pressure builts until it has to escape someplace.
