I have a 2008 Mitsubishi Outlander XLS V6
6 Speed Transmission it currently has
120,000 miles the transmission fluid has
never been changed, should I change it
now or leave it alone because it will cause
immediate failure after?

Yes change it and change it again in a few thousand miles. The filter too.

my advise is to change it to have a peace of mind of failure occur in the near future
