Out of balance tires is my first guess but the tires could be out of round, the wheel bearings loose or the front suspension worn and out of alignment. You also might need new shocks.

Tires should be rotated every 7k miles for best tread wear and longevity of tires. You could need alignment, balance if you have not recently done either of those. I would start there and have rotors checked / resurfaced / replaced as needed before. Could be wheel bearing needs repacked / replaced.
