Any codes? Run a compression test and look at the spark plugs while your there one maybe cracked.
Any car that's badly out of tune will do that; especially if it has old, fouling plugs. I'm guessing you also have misfires and maybe even a CEL for that. Just do a good tune-up and that'll likely fix it.
