The hose clamp holding the hose on is not tight enough or is somehow broken and not tightening properly. Put a new clamp on it. Make sure its nice and tight, then double check that all the air is actually out of the system and all should be well. A new radiator cap like Dave suggested can't hurt, but that's not the problem.
