Sometimes it has trouble pulling itself other times it doesn't

U go down the road and it just starts revving up
then it will catch and go and some kind of noice
coming out from the bottom

If the truck is revving up like you said and the engaging, that sounds transmission related to me. A symptom like that can be as simple as a low transmission fluid level to internal damage. Have a transmission mechanic diagnose the issue. Any lights lit up on the instrument cluster like the check engine light? If so scan for the trouble codes.
