1998 Ford Explorer Sport has started overheating

temp gauge goes to the top when driving freeway for length of time..noticed it will run warm but jump to
Hot when going uphill.  Radiator overheats and water boils out.  Tonight ran for 20 minutes highway in
mid to slightly above mid temp, then when was stopped at signal jumped to Hot...then dropped once
moving again.  Also noticed in past 2 months before overheating that on cold days when I had the heat
running it seemed it was not blowing hot air.

thermostat, and rad fill berp all air out, its low now or a air pocket going around for no heat
