2005 Subaru LGT chugs and shifts hard

Put the car in reverse after start up and it died on me.
After starting again the gears were shifting hard, and car
would start chugging. Saw some smoke coming out the
back exhaust mainly when going up hill. Chugging is
worse when I try to speeed up, and it feels like I’m losing
overall power. Need help ASAP!

There are a few possibilities of what the problem is. It sounds like you're getting oil into your combustion chambers and fowling out your plugs along with mixing with your gas causing the smoke. The problem could be the head gasket, the head, your piston rings, or the EGR.
