Transmission trouble

I have a 1994GMC Vandura 3500 with a 5.7 L V-8, and automatic trans.  Just today on the way home, the transmission started jerking very hard shifting between 1st and second gear.  Anyone have any ideas on what is causing that?  I'm afraid to drive it now.  It shifts from 2nd to third and third to 4th with no problem.  Checked the fluid and it's clean and full, and I had it serviced 18,000 miles ago.  Please help
