Well, to give you an idea, my 2001 Chrysler 300M, 6 cyl gives me 22-28mpg. My 2008 Mustang GT, 8 cyl, gives me 22-26 mpg (Stickshift). So I think your little engine is working hard to move a heavy body. In other words, the ratio between the horsepower and the weight of the vehicle is not well balanced. You can try changing the differential to one that is more direct. It will reduce your torque but will increase your MPG. I used to have a Toyota forerunner, 6 cyl, and was getting 15 MPG! Too much weight, too little engine.

I just purchased a '96 4 cyl 5 speed S-10 and get 22 city and 28 highway (65 MPH) This is close to the factory numbers for this truck and I'd check tune-up stuff and forget about changing gear ratios. 118 HP is plenty for this truck unless you tow heavy loads- there was a 6cyl engine option for that.
