2001 Highlander six cylinder performance issue

My Highlander has 430 000 km with about 250 000 on a replacement six
cylinder engine.  When on cruise...and sometimes off cruise it hesitates when
going down hill or on the flat.  This can last for 5-10 seconds.  The odometer
and rpm do not change but their is a slight clunk.

An addendum to the above. Have done regular maintenance but have never changed the plugs or timing belt.
