Mazda CX-5 accident - roll back on hill

Parked on a hill I was dropping off my friends
and I put the gear in park with the engine
running and helped them get their gear out of
the trunk.  I then leaned into the car which
was still in park and turned off the ignition.  
The car then started to roll back while still in
park making a very loud clicking sound. I
managed to jump into the car barely missing
a parked car that was side swiped. The car
was traveling at speed.  I tried to activate the
ebrake but it wouldn't activate.  My CX5
crashed into a second car which caused my
vehicle to stop.  Any ideas what happened?

The park pawl did not engage fully. This could be a warranty/recall issue. Of course they will blame you for not setting the e brake.

I have just had a similar issue on my Mazda CX5 GT Sport which has done exactly the same thing - got home from work put it in park mode with engine running as on a phone call - opened drivers door ready to get out - noticed switch off engine button (stop/start) pressed it and the car then started to roll back and it bent my door fully back as it hit the garden wall and post before rolling into the main road in front of an oncoming vehicle which lucky stopped - I have just reported this to Mazda so lets see what happens now - it needs new door and repair to bodywork - but this park issue needs needs sorting asap Brian Pennington - 24/10/20

Today I watched as my daughter arrived at our house in her 2018 CX-5 GT, parked on our slightly sloped driveway, put it in park and shut off the engine. When she got out of the car to get her toddler out of the back seat, the car started rolling backward into the street. Luckily it stopped on a snow berm and my daughter didn’t get hurt.
