What is this item called it was removed while installing a
starter

That is a wire terminal with the wire cut off. Maybe someone left the old one on when replacing the power wire.

If your car is still turning over and running well, then that is a old throw away part, nothing to worry about. Or maybe keep it as a reminder of :-)
