So all of a sudden I'm driving and from the middle to the
back of the truck starts shaking real bad while running
and I don't know what's wrong what could it be?

The UJoints could be bad and the drive shaft is out of alignment. This condition will be worst on a flat road. Roll down window while driving on a flat road and listen for squealing noise coming from the underside of truck.
