I have a 1999 extended cab Chevy S10 and I was
wondering if a cold air intake would up the gas
mileage any or what else it might do to it?

Just keep it well tuned, filters clean and tires at their proper inflation, swapping out an intake might give you slightly better gas mileage but the return may be negligible.

I was told it makes an awful sound going down the road it helps but I don't think it's enough to justify it, k&n reg air filter helps too
