Could be lack of coolant flow; worn water pump or restricted radiator. An IR thermometer can tell if a radiator is uniformly hot, and many auto parts stores will lend or rent one out. If the radiator has cool spots then coolant flow is restricted and you may get overheating when driving slow or stopped.
