Chrysler Pacifica overheating

My 2006 Pacifica is over heating even
when the radiator fan is on .
  Is this a thermostat issue , water
pump issue , or something else ?

Start with the coolant level. If it's at the proper level, the possibilities are your thermostat, water pump, or system needs to be flushed. On diagnosing it, start with the simpler and cheaper components concerning the issue.
